Strive to Thrive Therapy provides mobile Occupational Therapy for adults across Greater Brisbane, including Brisbane’s northside and southside, Ipswich, Logan, Redlands and Moreton Bay.
Our services include Functional Capacity Assessments, Assistive Technology Assessments, Home Modification Assessments and Capacity-Building Occupational Therapy. Each pathway answers a different main question.

When might Occupational Therapy help?
Occupational Therapy looks at how a person manages everyday activities and how their abilities, routines, equipment, supports and environment affect participation.
Occupational Therapy may involve a focused assessment, a formal report, practical recommendations or ongoing therapy.
Occupational Therapy may help when:
- An everyday activity has become difficult, unsafe, tiring or more dependent on assistance.
- The person’s circumstances, health, confidence or support needs have changed.
- Current equipment, strategies or the home environment are no longer working well.
- A broad and current understanding of everyday function or support needs is required.
- The person wants to build or maintain practical skills, routines or confidence over time.
- The person, family or support team needs clearer direction about practical next steps.
Our Occupational Therapy Services
The type of OT support a person needs will depend on what is happening for them now, what they are finding difficult and what they need to achieve.
Our Occupational Therapy services are designed around different assessment and therapy needs. The questions below can help identify which service is most relevant to the reason for referral.
Functional Capacity Assessment
How is everyday life working now, and what support is needed?
An FCA builds a broad, current picture of everyday function, strengths, barriers and support needs, then provides a formal report with practical recommendations.
Assistive Technology Assessment
Could equipment or technology help with an activity, mobility need or safety concern?
Assessment considers the person, activity and environment and may include trials, supplier liaison and supporting documentation.
Home Modification Assessment
Is the physical home creating a barrier to access, transfers or everyday activities?
Assessment looks at the person, activity and home together to identify practical changes and clear next steps.
Capacity-Building Occupational Therapy
Would ongoing OT help the person work towards an everyday goal?
Therapy helps the person build, strengthen or maintain practical skills, routines, confidence and participation over time.
When more than one OT service may be needed
Sometimes one OT service is all that is needed. In other situations, an assessment or therapy may identify another area where Occupational Therapy support could be helpful.
An assessment can identify other needs
A Functional Capacity Assessment may identify a need for assistive technology, changes to the home environment or ongoing Occupational Therapy support.
A new solution may take time to put into practice
New assistive technology or changes to the home can sometimes create new ways of doing things. Ongoing OT may help the person get used to the change, work out new routines or strategies, and build confidence using it in everyday life.
Needs can change over time
During Capacity-Building Occupational Therapy, new needs may emerge over time. This could include a need for assistive technology, changes to the home environment or a more comprehensive assessment of current function and support needs.
We keep the initial service focused on the reason for referral. If another area of OT support is identified, we will discuss this with the person and relevant referrer before any additional work begins.
How mobile Occupational Therapy works
Strive to Thrive Therapy provides mobile Occupational Therapy across Greater Brisbane. Most appointments take place in the person’s home or another relevant community setting, giving our OTs the opportunity to see how the person manages everyday activities in the environments where they actually happen.
See how everyday activities play out
Being in the person’s own environment allows the OT to see what actually happens during everyday activities, including what is working well, where things become difficult and what factors may be contributing.
Work with the person and their team
We work with the person to understand what matters to them and what they want to be able to do. Where appropriate and with consent, family members, support workers and other professionals can also be involved to help build a fuller picture of everyday life and support needs.
Use what we learn to guide recommendations and therapy
What we learn during the appointment informs our recommendations and OT intervention. This may include practical strategies, changes to the environment or equipment, recommendations within a report, or an ongoing therapy plan.

We work with adults aged 18 and over who are experiencing difficulty with everyday activities, independence, safety or participation due to disability, health changes or other support needs.
Current funding pathways
We currently support private paying, self-managed and plan-managed NDIS participants, people accessing Support at Home, and adults requiring Occupational Therapy assessment to support an NDIS access request.

Do I need to know which Occupational Therapy service to request?
If you know which OT service is needed, you can select this when completing the referral form.
If you’re unsure, tell us what the person needs support with and what you are hoping OT can help with. You can also contact our team before referring if you would like to discuss the most appropriate service.
Can one referral involve more than one Occupational Therapy service?
Yes. Sometimes more than one area of OT support may be relevant. We keep the initial service focused on the reason for referral and will discuss any additional assessment or therapy needs before further work begins.
For example, an assessment may identify a need for assistive technology, home modifications or ongoing capacity-building OT.
Does Occupational Therapy always happen at home?
Typically, but not always. Many appointments take place in the person’s home because this allows the OT to see what actually happens during everyday activities and how the environment, equipment, routines and supports are influencing function.
Appointments may also take place in the community, at an equipment supplier or via telehealth when this is more appropriate for the assessment or therapy being provided.
Who can make a referral, and who can be involved?
A referral can be made by the person themselves, a family member, Support Coordinator, health professional or another person involved in their support. With the person’s consent, family members, support workers and other professionals can also be involved where their input is helpful.
What funding pathways does Strive to Thrive Therapy currently support?
We currently support private paying, self-managed and plan-managed NDIS participants, people accessing Support at Home, and adults requiring Occupational Therapy assessment to support an NDIS access request.
Strive to Thrive Therapy is not currently an NDIS registered provider. If you have NDIA-managed or mixed funding, contact our team to discuss whether we can assist.
What happens after a referral is submitted?
Our team reviews the referral, including the reason for referral, location, service requested and any relevant timeframes or clinical needs. We will then contact the person or referrer if we need any further information or to confirm the next steps.
We aim to make initial contact within two to three business days.

Availability varies depending on the person’s location, the OT service required and current clinician capacity. Contact our team if you would like to check availability before submitting a referral.
