If you’re supporting someone who needs functional OT support, this page guides you through who we help, how to refer, and the right form to use.
Who We Support
We provide mobile, in-home occupational therapy for adults living with:
- Physical disabilities
- Intellectual disabilities
- Psychosocial or mental health challenges
- Long-term functional limitations
- Reduced independence in daily living
- Mobility or safety concerns at home
We currently support:
- Self-managed NDIS participants
- Plan-managed NDIS participants
- Support at Home recipients
- Adults seeking help with an NDIS access request
Our work is person-centred, evidence-based and focused on increasing safety, confidence and independence at home.

How to Refer
Referring to Strive to Thrive Therapy is simple.
Step 1
Choose the form that matches the person’s funding pathway.
Step 2
Provide as much detail as you can. This helps us triage appropriately and begin planning support.
Step 3
Our team will contact you or the participant within 2 to 3 business days.

Find the Right Referral Form
Choose the referral path that best matches the person you are supporting.

NDIS Participant
Use this form if the person is already an NDIS participant and needs OT support.
We currently support self-managed and plan-managed participants.

Support at Home Recipient
Use this form for people receiving Support at Home who require in-home OT support.

NDIS Access Request
Use this form if the person needs OT support to help with their NDIS access request.
