NDIS
If you think you need NDIS support, there are a few things to consider and ways to gain access. Occupational Therapists are listed as one of the health professionals that are most appropriate to provide functional assessments and evidence of disability.

- Contact your local NDIS office and get in touch with a Local Area Coordinator
- You will be asked to provide evidence of your disability. You can reach out to an OT to complete a the necessary assessment and provide supporting evidence for your Access Request. For more information: https://www.ndis.gov.au/applying-access-ndis/how-apply/information-support-your-request
We are not currently registered with the NDIS. This means that we are only able to see clients where their Improved Daily Living funding is self or plan-managed.

Occupational Therapy
An Occupational Therapist (OT) can help people of all ages, from children to older adults. Their primary goals are to support their clients to achieve independence, engage in their meaningful occupations and improve their quality of life.
OTs assess a person’s abilities and the difficulties they have in performing their daily activities and achieving their goals. They develop individualised intervention plans to suit each client’s needs and goals and work with the client, their family and support network to achieve these goals.
Occupational Therapy helps you do the things that are important in your daily life. Our Occupational Therapists work with you to make everyday tasks easier and improve your skills.
Our goal is to help you become more independent and enjoy your daily activities. We create a personalised plan based on your goals and needs. Our therapists can support you at home, school, or work, ensuring you get the help where you need it most.
If you think you may need an OT, reach out to our team and we will be happy to talk to you about your concerns.
Everybody we meet has individual needs so there is no general timeline for how long you may need to see an OT. Our mission is to support our clients to lead as safe and independent life as possible to the point where OT intervention and services can be phased out or ceased completely.
If you are a NDIS client, you do not need a referral. You, your support coordinator or family member are able to complete the referral form on our website and we will be in contact. You can find our referral form here: www.strivetothrivetherapy.com.au/refer-to-us
If you are a DVA client, you will need your GP to complete the D904 form and send this form to [email protected]. You can find a copy of the D904 form here: https://www.dva.gov.au/about-us/dva-forms/d0904-dva-request/referral-voucher
If you are a Home Care Package client, please contact your Home Care Package Provider and advise that you would like to engage a Strive to Thrive Therapy Occupational Therapist. Alternatively, you can provide us with your package provider contact information and we will be in touch.
If you are a private paying client, you will not need a referral. Please get in touch with us directly to discuss your needs.

The first thing to do is reach out to our friendly team through our online contact form. We are more than happy to speak with you about your situation or concerns and guide you in the right direction whether that’s with us or with one of our trusted partners.
The first time you see an OT can be daunting. The focus of the first OT session is to always understand the client as a person. There are typically lots of questions asked and topics discussed during the session as part of the assessment process, some one which may be difficult to answer. We highly encourage having a support person attend the appointment with the client.
Our assessment sessions help the OTs to gather information to support a Functional Capacity Assessment and report.
A functional capacity assessment (FCA) is a holistic assessment that involves the OT, the client and their supports. It provides valuable insight into a client’s overall functional and their daily support needs. A report is then provided with the assessment outcomes and recommendations to support the client’s personal goals, maximise independence and promote safety and wellbeing in their home.
The NDIS highly values Occupational Therapists FCA reports to assist them in making their decisions. Get in touch with our team to discuss how an FCA may help you.
We are a mobile therapy provider, so our sessions take place at various locations to suit your individual needs. Sessions are typically conducted at your home to help us understand you in your environment. We may also see you at your place of work depending on what outcomes we are trying to achieve.
One of the key benefits of us coming to you in your home is that we are able to assess our clients within their typical environment. This allows us to see exactly the nature of their difficulties within their day to day routines and their familiar environments, to create the most appropriate plan and recommendations.
There is clinical evidence to indicate that the skills developed in therapy provided within the home can also support greater transfer of skills from therapy into day to day life and routine.
Our Practice
OTs at Strive to Thrive Therapy support clients with varying diagnoses such as those with physical limitations, cognitive limitations, mental health diagnoses or intellectual impairments.
If you find that your disability is impacting how you live your life or if you are the family member of someone with a disability and want to see them become more independent and reach their full potential, our OTs are able to lead them in the right direction.
We also support the aged community through the Home Care Package funding scheme.

Strive to Thrive Therapy only provides Occupational Therapy services. However, we believe that a collaborative and team approach is necessary in supporting our clients to achieve their goals. We have strong relationships with a number of other allied health professionals from other disciplines, such as psychologists, physiotherapists and exercise physiologists.
Rest assured that we are more than happy to join your existing team and work collaboratively to meet your needs.

You do not need to be an NDIS participant to see one of our therapists. In addition to the NDIS, we offer our services to private paying clients, Home Care Package clients and DVA clients.
We only provide Occupational Therapy services. However, we work closely with wonderful NDIS support coordinators and are able to support you to find your fit where required. Contact us for more information.
