Key Responsibilities
Work closely with Support Coordinators and other industry professionals to continue to build our strong network of supports for clients.
Uphold our mission to make a real difference in a client’s quality of life.
Conduct a variety of assessments, including Functional Assessments, Assistive Technology, Home Modifications and Therapies.
Design tailored plans to address each client’s unique challenges and goals.
Have a commitment to turn up daily for yourself, your clients and those around you
Take ownership of your place in the team and actively contribute to the growing interests of the team and company
Understand that clear and effective communication is the key to our success
